Bid Submission Location: Office of the Procurement and Supplies Officer, University Teaching Hospitals – Eye Hospital.

Required Document: Bids must be accompanied by a signed Bid Securing Declaration Form in a format provided in the bidding document.

Document Access: A complete set of Bidding Document in English language must be obtained from Procurement office, upon producing a GRZ Receipt Verified by accounts department. This receipt will be issued by the cashier upon payment of a non-refundable fee of One Thousand Kwacha (ZMW 1,000.00) to our banking agent located in the Outpatient Department.
