JOB PURPOSE: Undertakes implementation and coordination of comprehensive and sustainable HIV Prevention and Testing Services, linkage to ART treatment and Care, linkage to cancer treatment, and implementation of differentiated service delivery models to surrounding Community Posts, communities under their care and community IP at facility level.

REPORT TO: Facility In-Charge

PRINCIPAL ACCOUNTABILITY (MAIN DUTIES)

  • Undertake coordination of TB/HIV facility and community planned activities to ensure comprehensive and sustainable facility HIV program.
  • Undertake community mobilization to create demand for HTS, PMTCT, TB, and Cancer screening.
  • Provide school health services targeting adolescents and school-going children.
  • Undertake linkage of negative clients to HIV Preventive services such as PrEP, PEP, and other HIV preventive services.
  • Provide HIV Testing Services to all eligible clients under supported DSD modalities.
  • Ensure newly diagnosed HIV positive clients are linked to appropriate HIV treatment services as per guidelines.
  • Obtain generated list of patients currently on treatment who are due for drug pick up, clinical visit, and viral load testing on a daily basis from M&E.
  • Ensure that all patients due for drug pick up, clinical visit, and viral load testing who did not show up are contacted/followed up daily.
  • Ensure that all patients defaulting treatment are actively followed up, their status determined, and brought back into care.
  • Ensure that high viral load RoCs are offered Enhanced Adherence Counseling (EAC).
  • Facilitate provision of community and facility-based models of differentiated service delivery.
  • Coordinate identification and referral of clients to community-based groups and collaborate with community Implementing Partners to establish ARV distribution points.
  • Provide mentorship to volunteers and collaborate with community IPs to strengthen HTS, Prevention, Index Testing, SNS, PNS, and Community Tracking.
  • Supervise CBVs/FBVs to undertake assigned TB/HIV related duties.
  • Undertake relevant register documentation and reporting on implemented activities.
  • Develop sectional and individual work plans to monitor and evaluate performance.

JOB SPECIFICATIONS/ REQUIREMENTS

  • Grade 12 full certificate or equivalent
  • Diploma in Psychosocial Counseling, Public Health, Social Work or any other related qualifications
  • Must be between 21 and 55 years
  • Two (2) years’ relevant work experience under the CoAg (added advantage)
  • Ability to write technical papers and reports clearly in English
  • Ability to communicate fluently in English and local languages
  • Other skills/Attributes: Computer literate, Managerial skills, Interpersonal relations and ability to use initiative
